How do I sign up?
In order to have a patient portal account you will need to already be a patient at this practice. To activate your account, you will need to receive an activation code from us. Once you have received the code, you will be able to create your own username, password, and other login information used to verify your identity. If you would like to activate your portal account, please contact the office at 724-283-3170 to receive your activation code.

Meet the Staff
Dr. Fozia A Chatta MD
Dr. Chatta graduated with her Medical Degree from Punjab Medical College in 1983. She completed medical training in Chicago and has been practicing in the Butler area since 1995. Dr. Chatta opened her own practice in March of 2013.
Emiley Neudorfer NP
Emiley graduated with her BSN from Duquesne University in 2013 and her MSN from Wheeling Jesuit University in 2018. She has 6 years of Critical Care Nursing experience.
Emiley has been a Nurse Practioner at the office since 2018.
Kirsten L
Office Manager, Medical Assistant
Kirsten has over 15 years experience in the medical field. She started as a medical assistant with Dr. Chatta in 2013, and assumed the role of Office Manager in 2015.
Connie B.
Medical Administrative Assistant
Connie has over 12 years experience in the Administrative Medical Field and been with Dr. Chatta since February 2020.
Stephanie M.
Medical Assistant
Stefanie has over 24 years of experience in the Medical Field and has been with Dr. Chatta since June of 2020.
